A Brazilian butt lift, or BBL, uses your own fat to add volume, projection, and a rounder shape to the buttocks. Fat transfer, also called fat grafting or lipofilling, is a well-established technique in plastic surgery.
The procedure has two parts. First, liposuction is performed on donor areas such as the abdomen, flanks, lower back, or thighs through small incisions. This sculpts the waist and hips, which on its own enhances the buttock contour. The harvested fat is then processed by settling, filtering, or washing to obtain clean fat tissue, which is injected in small amounts spread evenly so each portion is surrounded by healthy tissue that can nourish it.
Safety is the most important topic with a BBL. The most serious risk is fat embolism, which can happen when fat reaches the large veins inside or beneath the gluteal muscle. For that reason, current safety recommendations from plastic surgery societies call for placing fat only above the muscle, in the subcutaneous layer, never into or under it. It is a question worth asking any surgeon you consult, in Mexico or in the US.
Some of the transferred fat does not survive and is reabsorbed by the body over the first months. The fat that takes is considered permanent and behaves like fat elsewhere in your body, so it changes if you gain or lose weight.
A BBL is usually a good fit for people who want more volume and projection in the buttocks, or a more defined waist-to-hip curve, and who have enough fat elsewhere to transfer. It appeals to patients who would rather avoid implants and also want to reduce stubborn fat.
If you are very slim and do not have enough fat, your surgeon may suggest butt implants instead. If you are traveling to Mexico City, plan ahead for the sitting restrictions during your trip home.
Recovery
Bruising, swelling, and tenderness in both the liposuction areas and the buttocks are normal. Some fluid may drain from the incisions during the first hours.
You will usually be asked not to sit directly on your buttocks and to sleep on your stomach or side, so the grafted fat is not compressed. You will wear a compression garment over the liposuction areas.
Your surgeon will see you during the first week and again a few weeks later. Early on, your buttocks may look larger than expected because of swelling.
By around the third month, the fat that did not survive has been reabsorbed and the final result can be assessed against your before photos. A second session is sometimes considered.
